Volunteers

We greatly appreciate your commitment to transforming lives in our community. Our insurance policy requires that we have an accurate record of all volunteers. This agreement is to release New Beginnings of Central Florida, of all liability while working/volunteering with New Beginnings of Central Florida. This agreement is effective upon signing.This Release and Waiver of Liability (the “Release”) executed on the date below, by(“Volunteer”) in favor of New Beginnings of Central Florida, a Florida nonprofit corporation, their directors, officers, employees, and agents(collectively, “New Beginnings”). The Volunteer desires to donate their time and talent to New Beginnings by engaging in related activities (“Activities”). Activities may include construction and rehabilitating residential buildings, working in the New Beginnings’ offices, participating in special events and fundraisers, and events or projects at the New Beginnings’ housing complexes. The Volunteer hereby freely, voluntarily, and without duress executes this Release under the following terms: Release and Waiver: Volunteer does hereby release and forever discharge and hold harmless New Beginnings and its successors and assigns from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from Volunteer’s Activities with New Beginnings. Volunteer understands that this Release discharges New Beginnings from any liability or claim that the Volunteer may have against New Beginnings with respect to any bodily injury, personal injury, illness, death, or property damage that may result from the Volunteer’s Activities with New Beginnings, whether caused by the negligence of New Beginnings or its officers, directors, employees, or agents or otherwise. Volunteer also understands that New Beginnings does not assume any responsibility for or obligation to provide financial assistance or other assistance, including but not limited to, medical, health, or disability insurance in the event of injury or illness. Medical Treatment: Volunteer does hereby release and forever discharge New Beginnings from any claim whatsoever which arises or may hereafter arise on account of any first aid, treatment, or service rendered in connection with the Volunteer’s Activities with New Beginnings. Assumption of the Risk: The Volunteer understands that the Activities includes work that may be hazardous to the Volunteer, including, but not limited to, construction, loading and unloading, and transportation to and from the work sites. Volunteer hereby expressly and specifically assumes the risk of injury or harm in the Activities and releases New Beginnings from all liability for injury, illness, death, or property damage resulting from the Activities. Insurance: The Volunteer understands that New Beginnings does not carry or maintain health, medical, or disability insurance for Volunteers. Each Volunteer is expected and encouraged to obtain his or her own medical or health insurance coverage. Photographic Release: Volunteer does hereby grant and convey unto New Beginnings all rights, title, and interest in any and all photographic images and video or audio recordings made during the Volunteer’s Activities with New Beginnings, including, but not limited to, any royalties, proceeds, or other benefits derived from such photographs or recordings. Public Records: At any time during the tenure of volunteering, the volunteer hereby authorizes New Beginnings to procure an investigative report compiling information from public records, past or present employers, business or personal references, and any other source required to verify information voluntarily supplied. The volunteer may request a complete and accurate disclosure of the nature and scope of the background verification; to the extent such investigation includes information bearing on character, general reputation, personal characteristics, or criminal activity. Other: Volunteer expressly agrees that this Release in intended to be as broad and inclusive as permitted by the laws of the State of Florida and that this Release shall be governed by and interpreted in accordance with the laws of the State of Florida. Volunteer agrees that in the event that any clause or provision of this Release shall be held to be invalid by any court of competent jurisdiction, the invalidity of such clause or provision shall not otherwise affect the remaining provisions of this Release which shall continue to been forceable. Confidentiality Statement: It is the policy of New Beginnings to hold confidential all observations and information made by, between and about New Beginnings participants. Also, confidential are all conversations between staff members,interns, board members, and program volunteers. Furthermore, all personal information such as employment, residence, and phone number, address of participants, staff and volunteers are not to be disclosed except with the explicit written permission of the individual(s) involved. I agree to follow the New Beginnings policy of confidentiality. I understand that this agreement will be placed in an administrative file and that any violation of this policy may result in my release from all volunteer activity and/or legal action. IN WITNESS WHEREOF, person named below executed this Release effective the date written.
